Order Request Form

Read all information carefully. Provide all required details to ensure accurate processing and timely service.

Important Info - Applies to All Orders:

• One order form per uniform. Complete a new form for each style.

• Minimum order requirement: Six (6) uniforms per style, per order. Males are included (e.g., 5 girls + 1 boy is acceptable).

• Turnaround times are firm. Our current turnaround time is up to 7 weeks. We do not adjust timelines for event dates and do not accept rush orders. Please plan accordingly.

• All sales are final. Due to the custom nature of our products, we do not offer refunds or exchanges.

• Shipping only. We do not offer local pickup.

• No deposits. The full balance is due before production begins.
